ABSTRACT

The concept of ethnic medicine is divided into a broad sense and a narrow sense. The broad concept refers to the traditional medicine of the Chinese nation, and the narrow concept refers to the traditional medicine of Chinese ethnic minorities. The external medicine is one of the main forms of ethnic medicine, and it is also the important content of ethnic medicine for external use, which is widely used in clinical practice. As the theory of ethnic medicine is unique, the application methods have certain characteristics, which are the key technical parts of clinical practice. However, the existing traditional Chinese medicine consensus formulation me-thods cannot meet the needs of the consensus formulation of the external ethnic medicine. Therefore, the methods suitable for expert consensus on external ethnic medicine are required. This article took Expert opinion on clinical application of Baimai Ointment as an exa-mple, and explorde a reasonable, effective, multi-dimensional, and multi-stage method to formulate expert consensus on the external ethnic medicine. In this research, three-dimensional sources of information, including ancient classics, clinical research evidence, and expert application experiences, were systematically and scientifically collected. After organization and analysis, the information was formed into comprehensive evidence. In a formal consensus meeting, part of the recommendations reached consensus. As to the issues that did not reach agreement, in-depth interviews were used to explore the reasons for the differences and resolve the disagreements. Finally, unanimous recommendations were reached. There are common problems during the formulation process of Expert opinion on clinical application of Baimai Ointment. This study is expected to provide references for the formulation of expert consensus on other external ethnic medicine.

ABSTRACT

ObjectiveTo explore the effect of Jianpi Yichang power on the nucleotide-binding oligomerization domain-like receptor protein 3 (NLRP3) inflammasome signaling pathway in a rat model of ulcerative colitis (UC). MethodSixty Sprague-Dawley rats were randomly divided into a normal group (n=10) and an experimental group (n=50). The experimental group received 5% dextran sulfate sodium (DSS) solution freely for 7 days to induce UC, and then they were further randomly divided into model group, sulfasalazine (0.3 g·kg-1) group, and high-, medium-, and low-dose Jianpi Yichang power groups (54.4, 27.2, 13.6 g·kg-1) for continuous treatment of 14 days. The general condition of the rats was observed and recorded daily, and the disease activity index (DAI) was scored before and after treatment. Enzyme-linked immunosorbent assay (ELISA) was used to detect the levels of interleukin-1β (IL-1β) and interleukin-18 (IL-18) in the serum of rats in each group. Hematoxylin-eosin (HE) staining was performed to observe the histopathological changes in the colon tissue. Immunohistochemistry, Western blot, and Real-time polymerase chain reaction (Real-time PCR) were used to detect the positive protein expression, protein expression, and mRNA expression of NLRP3, apoptosis-associated speck-like protein containing a CARD (ASC), and cysteine aspartate-special proteases-1(Caspase-1) in the colon tissue. ResultCompared with the condition in the normal group, the general condition of rats in the model group was relatively poor, with increased DAI scores (P<0.01), pathological changes in the colon, increased levels of IL-1β and IL-18 in the serum (P<0.01), and enhanced positive protein expression, protein expression, and mRNA expression of NLRP3, ASC, and Caspase-1 in the colon tissue (P<0.01). Compared with the condition in the model group, the general condition of rats in the Jianpi Yichang power groups at various doses improved significantly, with reduced DAI scores (P<0.05, P<0.01), alleviated pathological changes in the colon as revealed by HE staining, and reduced protein expression levels of NLRP3 and Caspase-1 in the colon tissue (P<0.05, P<0.01). The serum levels of IL-1β and IL-18, and ASC protein expression in the colon, as well as the mRNA expression levels of NLRP3, ASC, and Caspase-1, decreased in the high- and medium-dose Jianpi Yichang power groups (P<0.05, P<0.01). The positive protein expression levels of NLRP3, ASC, and Caspase-1 were reduced in the high-dose Jianpi Yichang power group (P<0.01). The positive protein expression levels of ASC and Caspase-1 were reduced in the medium-dose Jianpi Yichang power group (P<0.05). The mRNA expression level of ASC was reduced in the low-dose Jianpi Yichang power group (P<0.05). ConclusionJianpi Yichang power can reduce colon immune inflammatory damage by regulating the NLRP3 inflammasome signaling pathway, thereby exerting a role in treating UC.

ABSTRACT

With the improvement of sanitation, the infection rate of hookworm is greatly reduced and the severe infected case is rarely reported. Combined morphological and molecular biological examinations, a severe hookworm infection patient was diagnosed in Department of Laboratorial Examination, Quanzhou First Affiliated Hospital of Fujian Medical University. The morphological methods such as direct fecal smear microscopy, saturated brine flotation and hookworm larvae culture methods were used to identify the eggs and larvae from stool samples of the patient. There were a large number of hookworm eggs in patient's stool samples, and the average count was 60 840 per gram by modified Kato method, which belonged to severe hookworm infection. Meanwhile, to distinguish the hookworm species, the semi-nested RT-PCR assay was employed to detect hookworm internal transcribed spacer series from eggs in patient's stool samples, and the result showed that the hookworm species was confirmed to be Necator americanus.

ABSTRACT

The present study aimed to systematically evaluate the efficacy and safety of Ginkgo biloba extract 50(GBE50) in the treatment of ischemic stroke. The databases including CNKI, Wanfang, VIP, SinoMed, PubMed, EMbase, and Cochrane Library were searched for randomized controlled trial(RCT) of GBE50 for the treatment of ischemic stroke reported between database inception and May 2020. The methodological quality of the included RCTs was evaluated via the Cochrane risk of bias tool. The RevMan 5.4 was used for Meta-analysis. Sixteen RCTs were included, involving 1 615 patients with acute ischemic stroke. Most of the included RCTs reported the methods of random sequence generation, but only two performed the concealment of random sequence. All RCTs failed in blinding. Two RCTs reported the information of cases lost to follow-up and drop-outs. Since the number was small, the baselines of groups remained balanced. All RCTs reported key outcomes of ischemic stroke, which made selective reporting bias in a low risk. Meta-analysis results revealed that GBE50 combined with routine therapies could effectively lower the score of the National Institutes of Health stroke scale(NIHSS) and restore cognitive function and daily activity in ischemic stroke patients. Compared with routine therapies, the combination is advantageous in treating patients with ischemic stroke. However, high-quality multicenter RCTs with large sample sizes are still required for verification.

ABSTRACT

PURPOSE: The purpose of this study was to explore the potential early diagnostic value of serum microRNA-381(miRNA-381) in patients with gastric cancer (GC). MATERIALS AND METHODS: Patients with advanced gastric cancer (AGC) and early gastric cancer (EGC), as well as healthy individuals, were enrolled in this study. Expression of miRNA-381 in serum was detected using real-time quantitative PCR. Electrochemiluminescence analysis was used to investigate the expression of classic tumor markers, including carbohydrate antigen (CA) 199, CA724, and carcinoembryonic antigen. Finally, receiver operating characteristic curve and Kaplan-Meier analysis were used to determine the value of miRNA-381 in clinical diagnosis of GC. RESULTS: miRNA-381 was differentially expressed among the study groups. AUC analysis showed that the sensitivity and specificity of serum miRNA-381 in the diagnosis of GC were superior to those of other tumor markers. Furthermore, low levels of miRNA-381 expression were positively correlated with lymph node metastasis and AGC. Finally, Kaplan-Meier survival analysis showed that down-regulation of miRNA-381 was associated with lymph node metastasis and the development of GC. CONCLUSION: miRNA381, which was down-regulated in GC, might be a novel early diagnosis marker for patients with GC.

ABSTRACT

Orchidaceae (Orchidaceae) is the second largest family of angiosperms. It’s the "flagship" group in plant protection. The existence of orchid plant is closely related to mycorrhizal fungi. The relationship between orchids and their symbiotic mycorrhizal fungi is a benefit to the protection and population restoration of orchids. Aims: The research was aimed to study molecular identification about 15 strains of mycorrhizal fungi from 6 plots by rDNA ITS technology in order to understand and utilise the mycorrhizal fungi of Liparis japonica (Miq.) Maxim. Study Design: The mycorrhizal fungi collected from different geographical locations were isolated and purified from the mycorrhizal fungi symbiotic with Liparis japonica in Northeast China, which were identified by rDNA ITS, meanwhile computed evolutionary distance and constructed the phylogenetic tree. Place and Duration of Study: In 2017, the root segments of Liparis japonica were collected separately from Qianshan, Changbaishan, Gaoguan, Guanmenshan, Dongling, Daqinggou. Methodology: Fifteen strains of mycorrhizal fungi collected from six plots were identified by rDNA ITS. Using DNAMAN software to analyse, the pairwise homology was compared by using the optimal global sequencing option. The evolutionary distances of fifteen strains were calculated by MEGA (Molecular Evolutionary Genetics Analysis) software package and their phylogenetic trees were constructed by neighbour-joining method. Results: With primers ITS1 and ITS4, the 15 mycorrhizal fungi strains of rDNA ITS got about 600 bp length. ITS length was about 582-613 bp, in which ITS1 length was about 177-190 bp, and ITS2 length was 246-273 bp. The mycorrhizal fungi strains were highly homology separated from one plot, mostly above 90%. The plots from the south to the north were as follows: Qianshan, Guanmenshan, Gaoguan, Dongling, Changbaishan, Daqinggou in China. Fifteen strains after separated and purified were identified to be the Epulorhiza of Orchid Rhizoctonia blasted with Genbank. The homology of the strains gradually decreased affected by the difference of the north and the south, namely there was an increasing trend of diversity from south to north. Conclusion: The homology of mycorrhizal fungi from one plot was higher because of the same soil environment and climate environment and so on, and strain type was single. Under the influence of microclimate in Northeast China, the homology of strains decreased gradually in the sample area, that is, the diversity gradually increased from the south to the north.

ABSTRACT

Objective To preliminarily validate the clinical usability of the ameliorated Kawashima Itch Scale(Xie-Kawashima Itch Scale) among adult pruritic patients on maintenance hemodialysis. Methods Xie-Kawashima Itch Scale was developed on the basis of Kawashima Itch Scale. Patients were asked to record their pruritus condition according to Xie-Kawashima Itch Scale or visual analogue scale(VAS) during daytime and night for two weeks. The record at the second week was used for analyzing the correlation between Xie-Kawashima Itch Scale and VAS. Results Totally 134 patients were enrolled in this study, among whom 128 entered the final analysis. Xie-Kawashima Itch Scale was positively correlated with VAS(r=0.832,95% CI=0.810-0.851,P<0.01 for daytime record;and r=0.848,95% CI=0.828-0.865,P<0.01 for night record). Subgroup analysis also showed similar correlations between different age groups and among different gender groups. Conclusion Xie-Kawashima Itch Scale has good correlation with VAS in patients on hemodialysis, without being affected by age or gender. Thus,it can be a useful tool for the assessment of pruritus in clinical practice and research.

ABSTRACT

AIM:To investigate the protective effect of Zhenrenyangzang decoction (ZRYZ) on the intestinal mucosal barrier function and the expression of tight junction proteins, zonula occludens-1 (ZO-1) and occludin,in the co-lon of rats with trinitrobenzenesulfonic acid (TNBS)-induced ulcerative colitis in order to explore the possible mechanism involved. METHODS:Male Wistar rats were randomly divided into normal group, model group, salazosulfapyridine (SASP) positive control group,high-dose ZRYZ(ZRYZ-H) group and low-dose ZRYZ(ZRYZ-L) group. Except the nor-mal group, the rats in other groups were given the TNBS/50% ethanol mixed solution by enema to make the ulcerative coli-tis model. The rats in positive control group were given SASP suspension liquid. The rats in ZRYZ-H group and ZRYZ-L group were orally administered with ZRYZ at doses of 30.4 g/kg and 15.2 g/kg, respectively, while the rats in normal group and model group were orally administered with saline. All the drugs were administered to the rats for consecutive 21 d. Disease activity index (DAI) was investigated during the drug treatment, and colon samples were collected after drug administration for evaluating morphological damage score. Intestinal mucosal permeability was measured by detection of lac-tulose/mannitol (L/M) in urine. Colonic myeloperoxidase(MPO) activity and goblet cell number, serum diamine oxidase (DAO) and D-lactic acid (D-LA) levels, and the colonic expression of ZO-1 and occludin were also measured. RE-SULTS:Compared with normal group,DAI,morphological damage score,L/M value, colonic MPO activity as well as se-rum D-LA and DAO levels in model group significantly increased(P<0.05),while goblet cell number as well as the ex-pression levels of ZO-1 and occludin in model group were significantly reduced(P<0.05). Compared with model group, DAI,morphological damage score,L/M value, colonic MPO activity as well as serum DAO and D-LA levels in ZRYZ-H group and ZRYZ-L group were reduced significantly(P<0.05),while goblet cell number as well as the expression of ZO-1 and occludin in ZRYZ-H group and ZRYZ-L group increased significantly(P<0.05). CONCLUSION:ZRYZ protects the intestinal mucosal barrier function in ulcerative colitis rats by reducing intestinal mucosal permeability, and its mecha-nisms may be related to increasing the expression of ZO-1 and occludin.

ABSTRACT

Objective To compare the consistency of thrombelastography (TEG) and light transmittance aggregometry (LTA) in monitoring the antiplatelet therapy of the patients with coronary heart disease (CHD) after percutaneous coronary intervention (PCI),and observe the changes of mean platelet volume (MPV) of the patients treated with aspirin and clopidogrel after PCI.Methods A total of 177 patients undergoing PCI and the treatment of aspirin and clopidogrel in Peking University First Hospital during March 2014 and May 2015 were enrolled in the study.Their adenosine diphosphate (ADP) or arachidonic acid (AA) induced platelet inhibition rates determined by TEG,MPV before and after antiplatelet therapy, and the maximum platelet aggregation rates measured by LTA from 99 patients were retrospectively analyzed. Results There was no any correlation between the maximum aggregation rates measured by LTA and the platelet inhibition rates determined by TEG regardless of using ADP or AA as agonist (all P > 0.05).The detection rates of clopidogrel hyporesponsiveness determined by LTA and TEG were 30.3% and 45.5%,respectively,while those of aspirin hyporesponsiveness were 19.2% and 31.3%,respectively.The detection rate of hyporesponsiveness determined by LTA was significant lower than that by TEG (P < 0.05).The MPVs after antiplatelet therapy were significant lower than that before treatment (all P < 0.01) regardless of clopidogrel hyporesponsive or sensitive and aspirin hyporesponsive or sensitive. The MPVs in clopidogrel hyporesponsive group before and after treatment were significantly lower than that in clopidogrel sensitive group (all P < 0.05).The PLT counts in clopidogrel or aspirin hyporesponsive groups after treatment were significantly higher than that before treatment (all P < 0.05).Conclusion There is poor correlation between LTA and TEG.It should be noted that the incidence rate of antiplatelet drug hyporesponsiveness is high in clinical practice. The MPVs of the patients significantly decrease after antiplatelet therapy. The patients with a significant increase of PLT after antiplatelet therapy are more likely to become drug hyporesponsiveness, while the patients with lower MPV are more likely to have clopidogrel hyporesponsiveness.

ABSTRACT

Objective To investigate the development strategy of novel T cell based vaccine against HCV infection. Methods BALB/c mice were primed with pSCK-based DNA vaccine and boosted with type 5 adenoviral vector-based vaccine, which expressed the structural proteins ( Core, E1 and E2) de-rived from a Chinese HCV patient (genotype 1b, Hebei strain).Enzyme linked immunospot assay (ELIS-POT) and intracellular cytokine staining ( ICS) were used to analyze the elicited antigen-specific immune re-sponses and the efficacy of cross-protection.Results Immunization of mice with the prime-boost vaccination strategy elicited stronger T cell immune responses against multiple HCV antigens than using the DNA vac-cines alone, especially the IFN-γ-secreting T cell responses against E1 protein as indicated by ELISPOT as-say.ICS data indicated that the prime-boost regimen elicited more TNF-α-producing CD4+and IFN-γ-produ-cing CD8+T cells against E1 protein and high levels of IFN-γ-producing CD4+and CD8+T cells against E2 protein in comparison with immunization with DNA vaccines. Moreover, the prime-boost vaccination was ca-pable of eliciting effective cross-protection in a surrogate challenge model based on a recombinant heterolo-gous HCV (JFH1, 2a) vaccinia virus. Conclusion The prime-boost vaccination using DNA and rAd5-based vaccine expressing HCV structural antigens induced significant cellular immune response and cross-protection in mice, suggesting the possibility of using it as a promising T cell based vaccine against HCV in-fection.

ABSTRACT

Objective To investigate the immunogenicity and cross protective effects of two novel HCV DNA vaccines in a mice model. Methods Two self-replicating alphavirus vector-based HCV DNA vaccines, pSCK CE1E2Y and pSCK H155, were constructed based on the genes encoding the structural pro-teins (Core, E1 and E2) and structural and NS3 fusion proteins (Core, E1 , E2 and NS3) of a HCV strain isolated from a Chinese patient (genotype 1b, Hebei strain), respectively.Western blot analysis was per-formed to detect the expression of fusion antigens. The BALB/c mice were intradermally immunized with the recombinant DNA vaccines by using electroporation. The immune responses induced in mice and the cross protective effects of the recombinant DNA vaccines were evaluated. Results The DNA vaccines effectively expressed the target antigens in vitro. The antigen-specific antibody responses and specific T cell immune re-sponses were induced in mice by the immunization of replicative DNA vaccines. However, no effective cross protection was provided by either of the DNA vaccines in the surrogate challenge model based on a recombi-nant heterologous HCV (JFH1, 2a) vaccinia virus strain. Conclusion Although no effective cross protec-tion was observed, both of the two replicative DNA vaccines could induce strong humoral and cellular im-mune responses against multi-target antigens of HCV strains. This study has paved the way for further inves-tigation on the development of novel HCV vaccines.

ABSTRACT

To develop a safe and broad-spectrum effective hepatitis C virus (HCV) T cell vaccine,we constructed the recombinant adenovirus-based vaccine that carried the hepatitis C virus truncated NS3 and core fusion proteins. The expression of the fusion antigen was confirmed by in vitro immunofluorescence and western blotting assays. Our results indicated that this vaccine not only stimulated antigen-specific antibody responses, but also activated strong NS3-specific T cell immune responses. NS3-specific IFN-γ+ and TNF-α+ CD4+ T cell subsets were also detected by a intracellular cytokine secretion assay. In a surrogate challenge assay based on a recombinant heterologous HCV (JFH1,2a) vaccinia virus, the recombinant adenovirus-based vaccine was capable of eliciting effective levels of cross-protection. These findings have im- portant implications for the study of HCV immune protection and the future development of a novel vaccine.

ABSTRACT

<p><b>OBJECTIVE</b>To compare the short-term clinical outcome between unilateral fixation fusion (ULF) and minimally invasive spine transforaminal lumbar interbody fusion (MIS-TLIF) in treating lumbar disc herniation (LDH).</p><p><b>METHODS</b>The clinical data of 39 patients with LDH were retrospectively analyzed from June 2008 to March 2013. There was 22 males and 17 females, aged from 45 to 75 years old with an average of 56.9 years. Therer were 3 cases in L3,4, 15 cases in L4,5, 21 cases in L5S1. Among them, 21 patients underwent unilateral fixation fusion (ULF group) and 18 underwent minimally invasive spine transforaminal lumbar interbody fusion (MIS-TLIF group). Operation time, blood loss, the times of radiographic exposure and hospital stay were noted and compared between two groups. Radiograph informations were regularily accessed and VAS, ODI scores were recorded at 3 days and 3, 6, 12 months after operation, respectively. According to modified Macnab criteria, the clinical effects were evaluated at final follow-up.</p><p><b>RESULTS</b>All operations were successful without severe complications. The averaged operative time and the times of radiographic exposure in ULF group [(95 ± 25) min and (4.2 ± 0.4) times] were less than that of MIS-TLIF group [(120 ± 35) min and (10.1 ± 3.9) times] (P < 0.05). But, the mean blood loss and hospital stay in MIS-TLIF group [(75 ± 45) ml and (7.2 ± 2.2)d ]were less than that of ULF group [(165 ± 60) ml and (11.0 ± 3.7) d] (P < 0.01). All patients were followed up from 12 to 45 months with an average of 29.5 months. The VAS and ODI score had significantly improved during the follow-up and no significant differences were found between two groups at the same time point (P > 0.05). The postoperative radiographs showed internal fixation position was good. And all patients obtained bone fusion by CT scan at 1 year after operation. There was no significant differences in modified Macnab criteria between two groups at the latest follow-up (P > 0.05).</p><p><b>CONCLUSION</b>Favorable short-term clinical effects can be achieved in suitable LDH patients with ULF or MIS-TLIF surgical procedures.</p>

ABSTRACT

We in the present research proposed a classification method that applied infomax independent component analysis (ICA) to respectively extract single modality features of structural magnetic resonance imaging (sMRI) and positron emission tomography (PET). And then we combined these two features by using a method of weight combination. We found that the present method was able to improve the accurate diagnosis of Alzheimer's disease (AD) and mild cognitive impairment (MCI). Compared AD to healthy controls (HC): the study achieved a classification accuracy of 93.75%, with a sensitivity of 100% and a specificity of 87.64%. Compared MCI to HC: classification accuracy was 89.35%, with a sensitivity of 81.85% and a specificity of 99.36%. The experimental results showed that the bi-modality method performed better than the individual modality in comparison to classification accuracy.

ABSTRACT

Objective To evaluate a new platelet function analyzer PL-11 with three major platelet function methods. Methods A randomized controlled trial was adopted.Totally 20 healthy young students from People's Liberation Army Medical School were enrolled in the study during July and August in 2013.Subjects were treated with aspirin or clopidogrel and the platelet function was measured by using of PL-11,as well as light transmittance aggregometry (LTA),VerifyNow and thromboelastography (TEG).Results When monitor aspirin response, correlations between arachidonic acid (AA) induced PL-11 and other methods were:LTA,0.614; VerifyNow,0.829; TEG,0.697,respectively.Biases between AA induced PL-1 1 and LTA were 1.94% at baseline and 24.02% during aspirin treatment.Cut-off value of aspirin response tested with AA induced PL-11 was 33.3%.When monitor clopidogrel response, correlations between adenosine piphosphate (ADP) induced PL-11 and other methods were:LTA,0.767; VerifyNow,0.851; TEG,0.608.Biases between ADP induced PL-11 and LTA were 3.01% at baseline and 6.56% during clopidogrel therapy.Cut-off value of clopidogrel response tested with ADP induced PL-1 1 was 66%.Conclusion Results of different platelet function testing methods were not equally effective.PL-11 has a high capability when monitoring short aspirin or clopidogrel response.
